Getting Started with Custom Forms

Custom forms are a powerful tool that allows you to collect the information that matters most from your campaign participants—whether they’re donating, registering, or engaging with your organization in another meaningful way.

 

What Are Custom Forms?

Custom forms give you complete control over the questions your participants answer when they take action. Whether you're collecting volunteer interest, accepting donations, or gathering sponsor details, you can tailor each form to suit your campaign’s needs.

You have two options to get started:

Use a Template: Choose from pre-built form templates designed for common campaign types. These can be cloned and edited to save time.

Start from Scratch: Build your form from the ground up to meet your specific goals.

 

Core Required Fields (Always Included)

To maintain platform-wide consistency and ensure accurate data mapping within the Activity Log, the following fields will be automatically included in every custom form.

First Name

Last Name

Email Address

Phone Number

Organization/Business/Family Name

These fields are essential for record-keeping and cannot be removed or duplicated.

 

Customizing the Experience

Beyond the required fields, you can add a wide variety of custom fields to your form, including:

Text fields (short or long answer)

Dropdown menus

Checkboxes

Date pickers

File uploads

And more!

Viewing and Managing Custom Forms

Once your custom forms are created, you can easily manage and review them from a centralized dashboard.

To view your existing custom forms:

  1. Navigate to Manage > Custom Forms
  2. From the grid view, you’ll see a list of all forms created in your account

Within this view, you can:

  1. Edit form titles to keep naming consistent and easy to identify
  2. Edit your Custom Forms 
  3. Preview forms to see what participants will experience
  4. See who created each form and who last modified it

This dashboard gives you full visibility into your form library and makes it easy to maintain consistency and update forms as your campaigns evolve.
