Using a Custom Form Template

Custom form templates provide a fast, streamlined way to launch forms with ease. This step-by-step guide will show you how to access pre-built templates and tailor them to your unique needs.


How to Use Custom Form Templates

1. Navigate to Your Custom Forms Grid

Select Manage > Custom Forms

 

2. Add New Form

Select Add Custom Form in the upper right corner. If you don't have any custom forms, you will have this action button in the center of your grid!

 

3. Search for a Template

  • Explore the available options under Choose From a Template.  
  • Preview the form to review the fields and content it captures. 
  • Hit Select Form next to the Form Template you would like to use.
  • Once you select the form, it will take you to the form editor where you can customize it.

 

4. Customize, Review, and Add

Add any additional fields by selecting them from the left-hand menu. You can also remove fields that aren’t needed, or simply click Add if the form is ready to go as is.
